-Childs name & age:
-Height/Weight/Teeth:
-DOB & Stats to be typed on the admitted one ticket:
-4-6 firsts or something else to be highlighted in middle:
-Quote under middle section:
-5-6 categories to be highlighted in outside boxes:
ANSWERS MUST BE SEMI EQUIVALENT TO SAMPLE SHOWN OR CUSTOM IS TO BE SELECTED. 

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
