-Name, Age:
Height/Weight/Teeth (older children may have these substituted for additional categories):
-3-6 likes/loves for the top two sentences:
-5 categories to be highlighted throughout the board with answers equivalent to those shown in the sample

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
