Childs name & Age:
Height/Weight/Teeth (for older children you may substitute these for additional categories):
12-16 likes/loves of the child:
4 categories to be highlighted on sides:
1 category to be highlighted on bottom:
*Answers must be equivalent in size to samples shown without selecting custom. 

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
