Childs name & Age:
Height/Weight/Teeth:
Date of birth, weight at birth:
10-15 likes/loves
4 categories to be highlighted (look at category samples above), with answers semi equivalent to samples shown (or pay custom fee). 

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
