Childs name & Age:
Height/Weight/Teeth (for older children you may substitute these for additional categories):
5-8 likes/loves of the child:
5-6 categories to be highlighted on the board with answers semi equivalent to those shown in sample. 

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
